Welcome to the Madison Family! We are glad you chose Madison School District.
Gather the documents below before starting a new enrollment.
- Student’s Birth Certificate
- Proof of residency - To enroll a student, the State of Michigan requires two items to prove residency:
- Mortgage, purchase or lease agreement, property tax document
- One (1) of the following
- Gas, electric, water, Internet or cable bill (dated within the last 30 days)
- Home insurance
- Up-to-date immunization record
- Most recent report card (applies to 1st - 9th grades)
- High school transcript (applies to 10th - 12th grades)
- Individualized Education Program (IEP), if services are required
- Proof of custody or guardianship, if student is not living with a parent
