Must be filled by and faxed to the HS Office at 517-265-1848 by Monday, May 10.

COVID Testing Consent Form

Students wishing to attend the Madison High School Jr/Sr Prom of After Prom on Saturday, May
15th, 2021, must produce a negative test result for Covid-19 on May 14th or May 15th*, 2021.
Rapid testing will be available at no cost to students on Friday, May 14th from 11:00am-noon
and from 4:00pm-5:00pm in the HS Gym. Students are not required to test at Madison High
School, but documentation of a negative test result shall be provided prior to admittance.
Testing must occur on May 14th or 15th.
